Does taking testosterone replacement therapy make you infertile?

Testosterone replacement therapy can reduce your fertility. So if having children is important to you now or in the future, it's worth discussing fertility preservation with your clinician before starting treatment. There are tweaks to your treatment plan that can make a big difference.

That TRT can negatively impact fertility might sound a bit odd. After all, men with naturally low levels of testosterone have lower sperm counts and quality than men with normal or high testosterone levels. So you might think that increasing your testosterone levels with TRT would improve fertility, right?

Unfortunately, it’s not that simple. Sperm production often falls when you take TRT.

For some men, this reduction is enough to make conception much more difficult. Others continue producing enough sperm to father a child naturally. Because everyone's response is different, TRT shouldn't be relied upon as a form of contraception.

How does TRT affect fertility?

To understand why TRT affects fertility, it helps to understand how sperm production normally works.

Your brain releases a hormone called gonadotropin-releasing hormone (GnRH), which stimulates production of two further hormones: luteinising hormone (LH), which tells your testicles to produce testosterone and follicle-stimulating hormone (FSH), which supports sperm production.

When you start TRT, testosterone is supplied using medication such as injections, gels or creams. While this can effectively treat symptoms of testosterone deficiency, it also changes your body's natural hormone signalling.

Although your blood testosterone levels increase, your brain interprets this as a sign that enough testosterone is already present and reduces the release of GnRH, LH and FSH. Without these hormonal signals, your testicles produce less sperm.

This is why TRT and fertility don't always go hand in hand. Even relatively low doses of TRT can suppress the body's natural hormone production.

Can you have kids on TRT?

Yes, it is still possible to have children while taking TRT, but your chances of conceiving may be lower.

Some men continue producing enough sperm to achieve a pregnancy naturally, while others develop very low sperm counts or stop producing sperm altogether. Because responses vary so much, there's no way to predict exactly how TRT will affect your fertility.

As you might still be able to conceive on TRT, it shouldn’t be considered a form of birth control.

Does TRT make you infertile permanently?

For most men, TRT does not cause permanent infertility. Once testosterone treatment is stopped, the brain gradually begins producing GnRH, LH and FSH again, allowing your testicles to restart testosterone and sperm production.

However, recovery isn't immediate. Sperm production typically takes several months to return and, in some cases, may take a year or longer.

How quickly fertility recovers depends on several factors, including your age, how long you've been taking TRT, the dose used, your fertility before starting treatment and any underlying medical conditions.

How to preserve fertility before or during TRT

If having children is important to you, there are several ways to help protect your fertility while treating low testosterone levels.

Ask about hCG

Human chorionic gonadotropin (hCG) acts in a similar way to luteinising hormone (LH), stimulating the testicles to continue producing testosterone despite the suppression caused by TRT. By helping maintain intratesticular testosterone levels, hCG may also support ongoing sperm production.

In some cases, specialists may also prescribe human menopausal gonadotropin (hMG), which provides follicle-stimulating hormone (FSH) activity if additional sperm support is needed.

Research shows that hCG can help preserve or restore sperm production in men receiving testosterone therapy, although individual responses vary.

hCG isn’t suitable for everyone and should always be prescribed and monitored by a specialist clinician experienced in male hormone health and fertility.

Freeze your sperm before treatment

If you know you'd like children in the future but don't plan to start a family yet, sperm banking before beginning TRT provides an additional safeguard.

Frozen sperm can be stored for many years and used later as part of fertility treatment if needed. While not everyone will require it, many specialists recommend discussing sperm freezing before starting testosterone therapy, particularly if future fertility is a priority.

Monitor your fertility

If you're taking TRT and hope to have children in the future, regular monitoring can provide reassurance and help identify any changes early.

A semen analysis is the most commonly used way to assess fertility, measuring factors such as sperm count, concentration and motility, though a normal result cannot confirm fertility — there are currently no tests that can tell you with certainty whether you are fertile or not.
