How to use your HRT products in hot weather

Heat affects the body's thermoregulatory system in ways that interact directly with the hormonal changes of menopause. It also raises practical questions about your treatment: Will my patch stay on? Should I apply gel before or after sunscreen? Is it safe to use a sauna? Can I take my HRT abroad without it deteriorating in a hot suitcase?

This guide works through the answers product by product, covering patches, gels, sprays, testosterone treatment, and vaginal oestrogen, as well as storage, swimming, holidays, and when worsening symptoms are a signal to speak to a specialist rather than simply wait for cooler weather.

Why your menopause symptoms can feel worse in the heat

The reason summer hits harder during menopause comes down to oestrogen's role in temperature regulation. A 2025 review published in Temperature confirms that vasomotor symptoms, including hot flushes and night sweats, are a thermoregulatory phenomenon driven by changes in hypothalamic KNDy neuron activity linked to oestrogen decline. In plain terms: declining oestrogen narrows the zone in which your body feels comfortable, making you far more sensitive to rises in ambient temperature.

A 2016 paper in Autonomic Neuroscience explains the mechanism further: oestrogen plays a direct role in how the hypothalamus regulates core body temperature. When levels fall, the thermostat becomes less stable, and external heat tips you over the edge more easily.

This is not a failure of your HRT. It is your body's biology interacting with the environment. What it does mean is that managing your treatment carefully in summer, so that it is absorbed consistently and stored correctly, becomes more important, not less.


Does heat affect how well HRT works?

The short answer is: normal summer heat is unlikely to significantly disrupt your treatment, provided you apply and store your products correctly. Extreme heat is a different matter.

A 2001 study in the Journal of Clinical Pharmacology examined transdermal patch hormone delivery across a range of conditions, including sauna, whirlpool, treadmill exercise, and swimming. Hormone levels remained within expected reference ranges across all conditions, and fewer than 2% of patches detached spontaneously. A 2016 study published via PubMed found similar results: oestradiol exposure during sauna, swimming, and whirlpool conditions was broadly equivalent to normal activity recordings for well-adhered patches.

What the evidence does support is this: a well-adhered patch applied to clean, dry skin is more resilient in typical summer conditions than many people fear. Where caution is warranted is in prolonged exposure to very high heat sources, such as saunas and hot tubs, where the rate at which hormone passes through the skin may increase. HRT patches in hot weather: what you need to know

Patches are the product most affected by heat and humidity, mainly because adhesion becomes more challenging when skin is warm and sweaty. Here is what to do.

Apply to clean, dry skin. This is the single most important step. Shower or wash the application site and allow skin to dry completely before applying the patch. Never apply to damp or recently moisturised skin.

Choose your application site carefully. The lower abdomen, lower back, and buttocks are the standard sites. In hot weather, avoid areas that are likely to become very sweaty (such as the waistband line under tight clothing) and rotate sites with each new patch to give skin a rest.

Avoid applying directly before sun exposure. Direct sunlight on a patch site can increase skin temperature at that location. Cover the patch with clothing when in direct sun where practical.

Press firmly for at least 30 seconds. Run your finger around the entire edge of the patch and press the centre firmly. This simple step significantly improves adhesion.

If your patch will not stick: try using a patch-fixing tape (such as Hypafix or Tegaderm, available from pharmacies) applied around the edges. Do not use any adhesive directly on the hormone-releasing membrane. If patches are consistently failing to adhere, speak to your specialist about whether a gel or spray might suit you better in warmer months.

Saunas and hot tubs: avoid them while wearing a patch. Prolonged immersion in very hot water or extreme heat environments may increase the rate at which hormone is released through the skin. While this has not been studied specifically in HRT patch users, the precautionary recommendation is to avoid saunas and hot tubs while wearing a patch. If you want to use one, remove the patch beforehand and apply a new one afterwards (check your product leaflet for guidance on minimum wear time).

Oestrogen gel in hot weather: timing and sunscreen

Oestrogen gel (such as Oestrogel or Sandrena) is absorbed through the skin and needs time to dry completely before anything else is applied over it.

Apply gel first, before sunscreen or moisturiser. This is not optional: applying sunscreen or moisturiser over wet or recently applied gel will affect how much oestrogen your skin absorbs. According to guidance consistent with NICE NG23 on transdermal HRT delivery, the absorption site should be left clear of other products until the hormone has been fully taken up by the skin.

Wait at least one hour before applying sunscreen or going swimming. This is the most commonly cited practical recommendation. Apply your gel in the morning and allow a full hour before sun cream, moisturiser, or water contact. If swimming or a beach day is planned early, apply your gel the evening before instead.

In very hot weather, consider switching to an evening application. Skin that is hot and sweaty may not absorb gel as reliably. Applying in the evening, when skin has cooled down, can support more consistent absorption.

Apply to cool, dry skin only. Just as with patches, avoid applying to skin that is still warm from the shower, damp from sweat, or freshly moisturised.

Do not apply sunscreen over the gel site and assume it is protected. Wait until the gel is fully absorbed, then apply sun cream to the area as normal.


HRT sprays and testosterone treatment in hot weather

HRT sprays (such as Lenzetto) follow similar principles to gel: apply to dry, cool skin and allow to dry completely before anything else is applied over the site. Sprays tend to dry more quickly than gel, but still allow a minimum of 60 seconds before dressing and at least one hour before water contact or sunscreen application.

The same rules apply: apply to clean, dry skin, wait before sunscreen or swimming, avoid applying to hot or sweaty skin, and store at the correct temperature (check your specific product leaflet). Vaginal oestrogen in hot weather

Vaginal oestrogen (pessaries, rings, or cream) is applied locally and is not affected by external heat in the same way as skin patches or gels. However, some practical points are worth noting.

Store pessaries and creams in a cool location, away from direct sunlight. In very hot conditions, some pessary formulations can soften; storing in the fridge (not the freezer) is acceptable and may actually make insertion more comfortable.

Swimming and bathing are not a concern for vaginal oestrogen in the way they are for patches or gel, as the product is not applied to an external skin surface. However, if you are using a vaginal ring, check your product information for specific guidance on swimming and activity.

Storing your HRT safely in summer

Incorrect storage is one of the most overlooked risks to treatment consistency during hot weather and holidays.

Check your product leaflet for the correct storage temperature. As a general guide, patches are typically stored between 15°C and 25°C, and many gels are stable up to around 25-30°C, but individual products vary. When in doubt, check the patient information leaflet or ask your pharmacist.

Never leave HRT in a hot car. Temperatures inside a parked car in summer can exceed 50°C within minutes. This is well above safe storage limits for any HRT product and can degrade the medication.

Keep products out of direct sunlight at home. A bathroom cabinet that receives direct afternoon sun, a windowsill, or a bag left on a sunny table are all risky. Store in a cool, dark drawer or cupboard instead.

Travelling abroad: Use a medication cooler bag or a small cool bag with an ice pack to transport your HRT safely. Many travel pharmacies stock purpose-made medication travel cases. Do not put products directly against an ice pack without insulation, as freezing can also damage some formulations.

If you think your HRT may have been heat-damaged: look for changes in the product's appearance (discolouration, separation in gels, damaged patches). If in doubt, do not use it. Contact your pharmacist for a replacement and check whether the medication needs to be reported as wasted.

Swimming, holidays, and saunas: your questions answered

Swimming with a patch: A well-adhered patch applied to clean, dry skin can generally withstand swimming for a normal period, as the pharmacokinetic studies referenced above suggest. Avoid long immersion in hot water, press the patch firmly before entering the water, and check its edges are sealed when you get out. If it has lifted, apply a patch-fixing tape around the edges.

Swimming with gel or spray: Wait at least one hour after application before swimming. If you are planning an early-morning swim, apply your gel or spray the previous evening.

Going on holiday: Do not stop your HRT treatment when you travel. Consistency of use is important for maintaining hormonal balance, and stopping and restarting treatment can disrupt symptom control. Plan your supply in advance: take slightly more than you need, store it correctly, and carry it in hand luggage when flying (not in a hold bag that may be exposed to extreme temperatures or pressure changes).

Saunas and hot tubs with a patch: As noted above, the precautionary recommendation is to avoid prolonged exposure to extreme heat sources while wearing a patch. Remove the patch before use and apply a new one after. The evidence on this is drawn from studies using contraceptive patches rather than HRT patches specifically, so the recommendation is precautionary rather than based on confirmed HRT-specific risk. When to speak to a menopause specialist

There is an important distinction between symptoms that feel worse because of hot weather and symptoms that are worsening because your treatment plan needs adjusting.

If your hot flushes and night sweats are temporarily worse during a heatwave and settle as temperatures drop, that is likely the thermoregulatory effect of heat on a menopause-sensitised system, and it is expected. If your symptoms are consistently worse this summer, are not settling between hot days, or you are experiencing new symptoms (low mood, disrupted sleep, joint pain, low libido), that is a signal to seek a review rather than assume it is just the heat.
