The difference did not reach statistical significance, likely because only a small proportion of the cohort was using HRT. The researchers acknowledge the study was preliminary and are calling for larger trials.

A 2025 narrative review went further, describing frozen shoulder as a neuro-endocrine-immunometabolic disorder. What the authors mean by this is that oestrogen changes involve body systems to weaken the body's defences in wide-ranging ways, predisposing women to frozen shoulder much more in midlife.

In other words: it's not just about one joint. It's about your whole hormonal environment.

Who is most at risk?

While the hormonal changes of menopause are a significant risk factor, several other conditions increase the likelihood of developing frozen shoulder:

Women aged 40 to 60 are the peak demographic. If you're in perimenopause or menopause, you're already in the highest-risk group.

Diabetes: People with diabetes have a 10 to 20% prevalence of frozen shoulder, compared to around 2% in the general population. Chronically elevated blood sugar appears to affect connective tissue in ways similar to declining oestrogen, increasing stiffness and inflammation.

Thyroid disorders: Both an overactive and underactive thyroid are associated with higher rates of frozen shoulder. If you have a thyroid condition alongside menopause, your risk is compounded.

The three stages of frozen shoulder

Treatment options

Frozen shoulder treatment requires patience. There are no quick fixes. But the right combination of approaches can significantly reduce pain and speed up recovery.

Physiotherapy

Physiotherapy is the cornerstone of frozen shoulder treatment. A systematic review found that joint mobilisation combined with stretching exercises produced better outcomes for range of motion and function than stretching alone. The key is consistency: regular, guided exercises over months, not weeks.

Your physiotherapist will adapt the approach to your stage: gentle maintenance exercises in the freezing stage, more intensive mobilisation in the frozen stage, and progressive strengthening as movement returns.

Pain management

Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or naproxen can help reduce pain and inflammation, particularly early on. Corticosteroid injections, up to three, may also be offered. These are typically most effective in the freezing stage, reducing pain enough to make physiotherapy more manageable. Heat or cold packs can help depending on the stage: ice in the acute inflammatory phase, warmth for ongoing stiffness.

For night pain, supporting your arm with a pillow and sleeping on the unaffected side can make a real difference.

Addressing the hormonal picture

This is where menopause care comes in. While physiotherapy treats the frozen shoulder itself, HRT addresses the underlying hormonal changes that may have contributed to it.

Can you prevent frozen shoulder during menopause?

Stay active. Maintaining shoulder mobility through regular movement such as swimming, yoga, or strength training keeps the joint healthy. Prolonged immobility is a known risk factor, so gentle, regular movement matters.

Manage diabetes and thyroid conditions carefully. If you have diabetes, good blood sugar control may reduce your risk. If you have thyroid dysfunction, make sure it's well managed.

When to seek help
