NHS Health Checks and physical activity, delivered out in the community
Am I eligible?
Most services are for people who live, work, study or have a GP in the borough.
Weight management: 18+, Hounslow resident, BMI 30+ — or 27.5+ for ethnic minorities or with co-morbidities
Stop smoking: aged 12+ and used tobacco in the past 48 hours
NHS Health Checks: aged 25–74 with no check in the last 5 years
