Estriol cream is a low-dose local vaginal oestrogen used to relieve the genitourinary symptoms of menopause, including vaginal dryness, soreness, irritation, and urinary symptoms. It is applied directly to the vaginal tissues using an applicator, and can also be used externally on the vulva. Estriol cream is suitable for postmenopausal women experiencing genitourinary syndrome of menopause (GSM), whether or not they are using systemic HRT.

What is Estriol cream?
Estriol cream contains estriol, a naturally occurring, weaker form of oestrogen, applied directly to the vaginal and vulval tissues to restore moisture, elasticity, and tissue health. Unlike systemic HRT, it acts locally with minimal absorption into the bloodstream, making it appropriate for a broad range of women, including those who prefer to avoid or cannot take systemic oestrogen. It is one of the oldest and most widely used forms of local vaginal oestrogen, available in the UK under brand names including Gynest and Ovestin. It is used daily for the first few weeks, then reduced to a twice-weekly maintenance dose.

Estriol cream provides targeted local relief for the genitourinary symptoms of menopause that are common, frequently progressive, and often undertreated. Because it can be applied both internally and to the external vulval tissue, it addresses a wider surface area than vaginal tablets or pessaries alone. Many women notice improvements in vaginal and vulval comfort within the first few weeks of use, with full benefits typically seen by two to three months of consistent use. A landmark double-blind placebo-controlled trial found that intravaginal estriol cream significantly reduced the incidence of recurrent urinary tract infections in postmenopausal women, alongside a marked reduction in vaginal pH and restoration of normal vaginal flora.
