Mounjaro needles: size, injection tips and what to expect

Mounjaro needle size is a common source of confusion for people starting on weight loss injections. Here’s what you need to know.

If you're starting Mounjaro (tirzepatide) for weight loss, it's natural to feel a little nervous about the injections—especially if you’re new to self-injecting. One of the most common questions we hear is about needle size.

Is it true that bigger is better for reducing discomfort, as some people are speculating on social media? Let’s break it down so you feel more prepared and confident before your first dose.

How long is the Mounjaro needle?

In the UK, Mounjaro is supplied in a KwikPen, which is a multi-dose pre-filled injection pen. Each pen contains four doses, but needles are not included in the box. That’s because Eli Lilly and Company—the manufacturer of Mounjaro—does not produce needles. Instead, the KwikPen is designed to work with a wide range of ISO 11608-2 compliant pen needles.

These pen needles vary in length and gauge:

  • Length: 4mm to 12.7mm
  • Gauge: 29G to 34G (the higher the number, the thinner the needle)

If you're using a private provider, such as Voy, your medication will usually come with the needles you need. At Voy, we supply four 4mm/32G needles with each Mounjaro pen, which are known for being short, thin, and well tolerated, so you’re ready to go from day one.

Why are shorter needles usually recommended?

While several needle sizes are technically compatible with Mounjaro, many healthcare professionals recommend shorter needles as they’re typically better tolerated, cause less pain, and lower the risk of injecting into muscle tissue (we’ll explain why this is important in a sec!).

In fact, a large clinical study comparing 4mm, 8mm, and 12.7mm needles found that 4mm needles caused the least discomfort with no increase in leakage risk—and were just as effective at delivering medication as longer ones.

So, what’s the bottom line? 4mm, 32G needles are generally a great option for Mounjaro. They’re short, thin, and widely tolerated—ideal for both new and experienced users.

How deep is Mounjaro injected?

Mounjaro is injected subcutaneously—that means into the fat layer just beneath your skin, not into muscle.

GLP-1 medications like Mounjaro are designed to be injected this way because the fat layer allows for slow, steady absorption, helping the medication work as intended.

If you accidentally inject into your muscle, the medication might absorb too quickly, which can lead to stronger Mounjaro side effects and possibly make the treatment less effective. Plus, injecting into muscle can be more painful and increase the risk of bruising or tissue damage.

For subcutaneous injections, shorter needles (like 4mm) are usually more than enough. You don’t need to pinch the skin or press hard. Simply insert the needle at a 90-degree angle straight into your skin until the wider base of the needle (called the hub) gently touches your skin, and hold it there for the full injection. This reduces discomfort and the risk of bruising or incorrect injection.

What about injection site reactions?

Injection site reactions are mild skin reactions that can happen where you inject. These can include redness, bruising, itching, swelling, a local rash, or pain. But such symptoms generally tend to settle within a few days and improve as your body gets used to the medication. In Mounjaro’s clinical trials, these reactions were common and usually mild.

Some users on online forums and TikTok have speculated that longer needles—like 6mm or 8mm—may reduce these reactions by delivering the medication slightly deeper, away from surface-level skin irritation. While this theory sounds plausible, there is no published clinical evidence to support the idea that longer needles reduce skin reactions or improve comfort.

In fact, using a longer needle increases the risk of injecting into muscle, which (like we just explained) can lead to faster absorption, stronger side effects and reduced effectiveness.

If you’re experiencing injection site reactions, they often improve with proper injection technique, rotating your injection sites, applying a cold compress, or using non-drowsy antihistamines.

But if you develop more serious symptoms—like worsening or spreading rash, swelling of your face, lips or throat, difficulty breathing or swallowing, or any palpitations—stop using the medication and seek urgent medical help immediately at your nearest A&E or through the NHS 111 service, as this may indicate a severe allergic reaction.

Does the Mounjaro needle hurt?

Understandably, many people worry about pain—but most find Mounjaro injections quick and very manageable.

Despite how it might look, the needle is very thin and the injection only takes a few seconds. In fact, many users report that they were surprised by how little it hurt—if at all.

One Voy user summed it up best:"I was dreading it, but honestly it felt like a quick pinch and that was it."

What matters most is good technique and staying relaxed. We’ll share more tips on that below.

Injection tips if you're nervous about needles

Here are some practical tips to help make your injections easier:

  • Familiarise yourself with the technique: Watch the video guide before injecting for the first few doses.
  • Use a fresh needle every time: Don’t reuse needles, even if they look clean
  • Choose the right spot: The stomach or upper thigh is often easiest to access
  • Rotate your sites: This helps reduce skin irritation
  • Let the pen warm up slightly to room temperature before injecting
  • Press the pen firmly and steadily: Keep it in place for the full injection time
  • Don’t rush: If you’re shaky or flinch halfway through, take a deep breath and try again with a fresh needle
  • Use a mirror if it helps you stay steady

And remember: practice makes perfect. The more you do it, the more second nature it becomes.

Can you buy replacement Mounjaro needles in the UK?

Yes—but you likely won’t need to right away. If you're receiving Mounjaro through a provider like Voy, your starter pack should include compatible pen needles and a sharps bin.

That said, if you ever need more, you can ask your local pharmacy for ISO 11608-2 compliant pen needles. These are usually prescription-only items in the UK, but some pharmacies may sell them over the counter if you’re already using injectable medication. Otherwise, you can ask your provider for a script.

Always dispose of your used needles and pens safely in your sharps bin. Never throw them in the general waste.
